Influence the policies, laws and other issues related to the practice of Case Management History 1999 The American Case Management Association is incorporated and becomes the first professional membership association for Hospital/Health System based Case Management 1999 National Case Management Week (NCMW) is announced 1999 ACMA establishes it's first state/regional chapter - ACMA Illinois Chapter 2000 1st Annual ACMA Meeting in Las Vegas, NV 2001 The First Leadership Forum for Hospital/Health System Case Management Professionals is held 2001 The First National Hospital/Health System Case Management Research Data is released 2003 The First Collaborative Case Management Publication for Hospital/Health System Case Management professionals is released 2003 ACMA, in conjunction with Joint Commission on Accreditation of Healthcare Organizations (JCAHO), establishes the “Franklin Award of Distinction,” which recognizes exceptional Hospital/Health System Case Management 2004 ACMA introduces breakthrough Avoidable Delay benchmarking program, Compare AD 2005 ACMA, in conjunction with Landacorp, a SHPS company, announces the establishment of the “Innovation in Case Management Service Award”, which honors a Case Management professional who exemplifies leadership in their practice through the development and implementation of innovative and creative improvements in Case Management service 2005 The First Hospital / Health System Case Management Certification, ACM™ - Accredited Case Manager, is developed by ACMA 2005 The First National Hospital/Health System Physician Advisor/Medical Director Forum is held 2008 ACMA announces partnership with Athena Forum 2008 ACMA announces partnership with Allscripts and ACS MIDAS+ to distribute Compare AD Definition of Case Management The ACMA's official definition of "Case Management," as approved by our membership in November of 2002, is as follows: " Case Management in Hospital/Health Care Systems is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self determination."
